I have read and understand the Borrowing Guidelines, including the return procedures of taking the gown to Hollywood Cleaners within two days of my simcha, paying them and sending the gemach a photo of my receipt the same day it is dropped off. I understand that the gemach will pick up my gown from the cleaners.

I also understand that once the gown leaves the premises the $75 fee is non-refundable and is considered a donation to the gemach.
